STEP ONE
Fill out the Appointment Request Form (we no longer schedule appointments over the phone)
STEP TWO
- Within 1 business day of submitting the appointment request form; our Intake Specialist will send you an email with the subject line: “American Healthcare Community Services welcomes you to the Patient Portal” (sent from email address Donotreply@alert.insynchcs.net).
- Click on the link in the email to access our online portal. (The portal works best in Google Chrome).

STEP THREE
Our Intake Coordinator will reach out to you for any clarifications, and then will assign you to the appropriate therapist. Our Intake Specialist will verify your benefits and what your patient responsibility will likely be. At the same time, our clinical staff reviews your request to confirm that our practice is the right fit for you.
STEP FOUR
You will receive a call from your therapist to schedule an initial appointment, after which you will receive an email confirmation of the appointment.
STEP FIVE
Follow the link in the confirmation email to login to your portal, complete the Primary Treatment Consent/Card Authorization Forms, and place your card on file.
STEP SIX
Once your forms are done, you are clear to have your initial consultation with your therapist.
